Please complete our self-referral form below. The more information we have, the better we can triage and can match you with the clinician who can help you best. All the information submitted is held confidentially. Once we receive a contact form we will make contact with you directly, usually within 48 hrs. GPs and specialists are also welcome to complete a referral or the GP can do this via the online referral tool, Specialist Referrals. We will still ask the family or adult to complete a contact form as well.
We are currently open to new clients across New Zealand with in-person appointments available in Auckland & Tauranga. Wait times can be between 2 and 12 weeks, depending on client location & presenting concerns. We can offer immediate support via a Single Session Intervention or One-Off Parent Consultation.
*Please note that we are currently only open for online referrals in the Wellington area.
